If you are a new or returning client, please complete these forms through the Client Portal at least 24 Hours prior to your first scheduled session so we have time to review. Otherwise this may result in the delay of starting your appointment on time.

If you are an active client, please feel free to access and review these forms at your leisure in your Client Portal.
